General Documentation

If we prepared your tax return last year:

  • Review the information request we emailed you
  • Review the other lists on this page for any changes in the last year!


If we did not prepare your tax return last year, please upload the following for you, your spouse, and any children or dependents:

  • Driver's license
  • Social security card
  • Previous federal & state income tax returns (previous 3 years)
  • Anything else that applies to you from the other lists on this page!


Homeowner Deductions

  • 1098 (Mortgage Interest)
  • Any other mortgage interest statement
  • Real estate & personal property tax payments
  • Receipts for energy-saving home improvements
  • 1099-S (Real Estate Sales)
  • Form HUD-1 for Real Estate Sales/Purchases


  • W-2 (Wages) 
  • W-2G (Gambling)
  • 1099-G (Government Payments)
  • 1099-MISC (Rents, etc.)
  • 1099-NEC (Contractor Income)
  • 1099 (Any other type)
  • Received alimony
  • Hobby income
  • Prizes and awards received


Healthcare & Medical

  • 1095-A (Health Insurance)
  • 1095-B (Health Insurance)
  • 1095-C (Health Insurance)
  • 1099-SA (Health Savings Account)
  • 5498-SA (HSA Contributions)
  • Healthcare insurance, doctor, dentist, hospital, etc. payment details
  • Miles driven for medical purposes
